| How to Amend an Issuer Event Report |
Note: An Issuer Event Report may be amended but cannot be deleted.
Steps on amending an issuer event report follow :

Click Issuer event report in the top navigation bar
The Form 55-102F4 screen appears |

Click Amend issuer event report in the left navigation bar.
The Amend issuer event report screen appears |

Click the radio button beside the report that you want to amend. Issuer event reports are listed by Effective date
Click Next |

The Amend and file information screen appears
Enter the new information in the fields
Click File and then click OK in the dialog box
The Amend issuer event report – Completed screen appears |
